We are seeking enthusiastic, motivated and reliable nurses who are keen to support and deliver a range of quality nursing services to our patients within Kirklees. You will have the energy, enthusiasm and drive to support our teams to deliver high quality services, as well as excellent professional skills.Main duties of the job Working within the Community nursing teams, you will undertake duties involving delivery of nursing services and holistic assessments of patient's health needs using a person-centered approach including providing quality and seamless nursing care to housebound patients and being proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings. In addition, you will have the opportunity to be involved in facilitating self-care for patients with long term conditions, health promotion, and complex out of hospital patient care. Desired Skills All Community nursing interventions for housebound patients, predominantly:

  • Holistic Nursing assessment and ongoing care planning liaising with the team and attendance at safety huddle. Medicines administration including Diabetes
  • Management (insulin), palliative care anticipatory medicines and syringe driver administration.
  • Frailty and OPAT Team Competencies
  • To provide holistic nursing assessments
  • To follow the escalation plan for the deteriorating patient
  • To provide patient centred care
  • Assume delegated responsibility for holistic assessment and the planning, implementation, monitoring and evaluation of programmes of care/ care packages, contributing to complex assessment/ care packages.
  • To help the patient towards self-care and maximising independence, maintain their optimum level of health and prevent further deterioration.
  • About us Essential for the role are- Clinical experience of IV therapy delivery, management of vascular access devices, experience in a community setting, venepuncture and cannulation skills and IV administration skills. We can offer bank shifts for you to book and choose at your convenience, training and support in practice to meet the requirements for registration and to meet personal needs. In particular, we are seeking to build our bank workforce with those who can only work weekends, evenings or Nights.
