Are you a newly registered occupational therapist or early in your career and wonder what a role working in the community is like? This is a fantastic opportunity to work in multidisciplinary teams offering holistic rehabilitation using a coaching and self-management approach to achieve patients goals. Our interventions are exciting and dynamic, supporting patient flow through urgent response and facilitation of hospital discharge with a Home First approach. You will also gain experience in core areas: Falls reduction and prevention work, case management of long-term conditions including neurology, MSK, orthopaedics, respiratory, rheumatology, frailty and palliative and end of life care.

Main duties of the job

The service functions over 7 days between 8am and 6pm. You will be aligned to the North, South or West hub area and we utilise a flexible working policy to accommodate needs where possible. A band 4 role may be offered if you await registration.

Supported by senior therapists you will be responsible for your own caseload delivering holistic rehabilitation or delegating to others, made meaningful by taking place in the home environment.

We are committed to your development, and you will work with the wider multidisciplinary team receiving regular clinical supervision, CPD opportunities and peer support. You will develop leadership skills supporting others including students and have opportunities to make a positive difference to both your patients lives and contribute to service improvements across the Trust and wider partners. We hope to expand the role into rotational opportunities in the near future as funding allows.
