Full Job Description

An opportunity has arisen for a dynamic and motivated Band 4 Community Public Health Associates (formerly referred to as a Nursery Nurse) to work within the newly integrated 0-19 Children's and Young People Public Health Service based in the East Locality covering Thame, Wheatley, Watlington and surrounding villages. The new service delivers the Healthy Child Programme (HCP) to provide an innovative and high-quality service that helps children, young people and families to thrive. The post holder will work without direct supervision to support clients within the universal and targeted aspects of the service provision. Health visitors, community public health nurses (formerly known as community staff nurses) and community public health associates will now be working with children up to 8 years of age and will provide health related episodes of care to support with areas such as behaviour, diet, toileting, communication and sleep. The service delivers the mandated contacts for the HCP, with the addition of a new four-year health and development review to support school transition and address any school readiness concerns. The service delivers health promotion groups, well baby drop in clinics and infant feeding support groups. Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required.Main duties of the job Within the caseload, and as delegated by the Specialist Community Public Health Nurse (SCPHN), the post holder will contribute to the safe and effective delivery of the 0-19 service. Working alongside the wider team you will promote children's physical and emotional health and wellbeing, identify health, social or developmental concerns at the earliest opportunity and contribute to the aim for all children to have the best possible start in life. You will be based within the locality (neighbourhood) and will work with the 0-19 teams in that locality and supporting county wide work where needed. Under the direction of the SCPHN (HV/SHN) the role involves working in partnership with children, young people, families, primary care, schools, statutory and voluntary services to facilitate health-enhancing activities and improve health outcomes. You will deliver services to children and families within homes, and community settings. Under the delegation of a SCPHN you will work with children 0-8 years and implement and evaluate episodes/packages of care according to the individual needs of the child/family. You will also run parenting groups such as "Early Days," and "Marvellous Me" and will deliver individual and group developmental reviews, and be involved, under supervision of the named SCPHN band 6, with complex situations (see detailed job description). The role will require the post holder to adhere to Safeguarding policies.
