Full Job Description
The post holder will provide direct care to patients with diabetes in hospital according to the needs of the patients and their families. They will work autonomously as well as within the multi-disciplinary team, in order to provide expertise and education to all health care professionals and carers with whom the patient with diabetes may come into contact during an admission to hospital.
They will be expected to support healthcare professionals in the delivery of excellent in-patient care, improving safety, reducing risk and promoting patient self-management where appropriate.
The post holder will be expected to manage their own caseload. If the post holder is a non-medical nurse prescriber he/she will be expected to prescribe new diabetes therapies, both oral and injectable, insulin, and initiate therapy in an in-patient environment and adjust therapies as appropriate.
Duties will include advice on peri-operative insulin regimes and insulin dose adjustments, in accordance with current protocols and to support ongoing evaluation, and development of all protocols relating to in-hospital diabetes care., As part of seeking to become more inclusive, we will consider requests for flexible working from the start of your employment. An exciting opportunity has arisen for a Diabetes Inpatient Specialist Nurses (DISN) to join the multidisciplinary Diabetes Team at York Hospital. The post holder will be required to support the multidisciplinary diabetes team in the management of people with diabetes, primarily in the hospital in patient serviced but also support the out patients department.
There is much emphasis on patient empowerment for self-efficacy in the management of long-term conditions, and therefore the post holder will require excellent interpersonal skills. They will need to be able to motivate and lead others and act as an effective role model. The ability to be flexible, creative and adaptable are key personal attributes.
Candidates will be expected to be able to demonstrate knowledge and awareness of the current clinical and professional issues affecting people with diabetes, incorporating high quality evidence in patient centred care. Additionally, they will demonstrate competency in teaching (patients and healthcare professionals), supervision of team members and sharing good practice. Experience of working in multidisciplinary teams is essential, while a post registration qualification in diabetes would be ideal.
