This is a part-time post for 33.75 hours per week (Monday-Friday). The Fracture Liaison Service (FLS) has an exciting vacancy for a dynamic and motivated individual to join a team working to improve outcomes for patients with osteoporosis and promote good bone health to the Royal Berkshire population. This post offers the opportunity to have a real impact on the independence and confidence of older patients in Berkshire. A passion for healthcare prevention is a must for this position. The FLS works alongside the Fracture clinic, Rheumatology, Radiology, Battle day unit, GP Practice's and all inpatient wards providing a varied work experience for the successful candidate., This post is responsible for supporting the Fracture Liaison specialist practitioners in developing and monitoring the pathways of care for patients aged 50 and over who have sustained fragility fractures. Other key responsibilities include organising and attending the FLS monthly meeting, tracking and telephoning patients as per fracture prevention pathway and protocol with the aim of identifying the first fracture to prevent the next. The post holder will ensure high levels of patient and clinician satisfaction by being an accessible, customer focused and knowledgeable point of contact. To provide a consistent approach across the Trust, using Standard Operating Procedures (SOPs) to ensure that functions of the role are carried out correctly within given timescales., The role of the FLS coordinator is integral to the Fracture Liaison Service as we develop and monitor the pathways of care for our group of patients. The postholder will have a pivotal role in enabling the Trust to deliver a high standard of care and communication throughout the fragility fracture patient's pathway. Effective tracking of the pathways will also facilitate the Trust in achieving CCG Commissioning Parameters, NICE recommendations, and national standards of care for patients with fragility fracture. The postholder will have responsibility for the organization of the FLS administration activity working closely with the Fracture Liaison Specialist practitioners. You will be responsible for the organisation and tracking of a defined group of patients across all departments to which they may transverse (Radiology, Rheumatology, Outpatients BIH and MBC). These patients may need to be identified from other sources and tracked. The postholder will also provide day-to-day intelligence about progress, troubleshooting blockages in patient pathways and providing proactive information in anticipation of potential problems, escalating as necessary., The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.

  • High level of computer literacy. Experience of using Microsoft Office, including Word and Excel
  • Excellent level of written and spoken English
  • Experience of working in a multi-disciplinary team
  • Customer Care experience and experience of handling patient complaints and queries
  • Desirable criteria
  • Experience in research database auditing and management
  • Willing to undertake further training in specialist area including the Foundation Fracture Prevention Practitioner Training, Excellent interpersonal skills with particular ability to liaise effectively with multidisciplinary team members
