What is Frailty?Frailty defines the group of older people who are at highest risk of adverse outcomes such as falls, disability, admission to hospital or the need for long term care.Frailty syndromes include:o Fallso Immobilityo Deliriumo Incontinenceo Side effects of medicationo People with frailty gradually lose their inbuilt reserves, leaving them vulnerable to dramatic sudden changes in health due to e.g. infection, medication change or a new environment, The Frailty Nurse will be a part of a team which comprises of MDT led by a Geriatrician/Acute Physician along with Social Services Therapies and Virtual Ward. This will deliver a comprehensive geriatric assessment at the front door within the Emergency Department, SDEC and will in reach into the Acute Assessment Unit and the Short Stay Unit Virtual Ward and the inpatient frailty wards They will also be linked to the integrated care in the community.
