Full Job Description

The Scarborough based GI Cancer Clinical Nurse Specialist (CNS) team currently has a vacancy for a full time Band 6 CNS. You will join a dynamic and welcoming team of 15 staff members.
Working in close collaboration within the team you will also build firm relationships with the Larger MDT including surgical, oncology, pathologist, and radiologist consultants. You will be supported and encouraged to broaden your spectrum of skills and knowledge of GI cancers and cancer pathways as you will be expected to act as a key worker for a defined caseload GI cancer patients.
In addition your role will involve -
+ Developing the service, identifying, and looking to improve ways of working through leading service improvements projects.
+ Supporting team members with Learning and development and carrying out 1:1's and appraisals for B4 members of the team.
+ Building relationships with other specialities locally and nationally, attending GI educational events.
+ Participating in the promotion of the GI team at Cancer Awareness events.
+ Working cross site as required to ensure the service is covered - at York Hospital

This post may become Permanent., Within your role you will work both face to face and remotely to -
+ Provide support for patients and their families on their journey through complex cancer pathways.
+ Attend and participating in MDT meetings, advocating for your patients, and through discussion ensuring next steps and outcomes are clear.
+ Undertake Nurse led clinics and work autonomously to provide information on GI Cancer Diagnosis to patients and their families.
+ Translate information into clear language to ensure patients are fully informed and aware of the next steps on their cancer pathway whether this be surgery, oncological treatment, or best supportive care.
+ Utilise specialist GI Knowledge to support your patients' holistic needs, considering their emotional, social, and psychological alongside any physical requirements.
+ Undertake regular reviews of patients on the caseload providing personalised stratified care, carrying out informed assessments, and writing care plans to ensure needs are met.
+ Refer patients to relevant specialities within the NHS and external providers for additional support.
+ Carry out end of treatment conversations with patients and arrange ongoing surveillance including requesting CT's, MRI's, bloods, and other investigations, based on patient assessment and need, As part of seeking to become more inclusive, we will consider requests for flexible working from the start of your employment.
