Full Job Description

  • lead delivery of the Healthy Child Programme for ages 0-5
  • conduct assessments and care planning
  • evaluate outcomes and support service improvement
  • champion health promotion and reduce inequalities
  • provide universal health reviews and targeted support to families
  • maintain NMC registration and work to The Code
  • safeguard children through collaboration and assessments
  • be the key contact and case holder for families.
  • You will need:
  • Qualification as a Health Visitor or nearing completion
  • NMC registration as a qualified nurse, midwife or allied health professional
  • passion for supporting children, families and public health
  • strong communication, interpersonal and listening abilities

    £40,215 - £45,595 per annum (pro rata for part time)

As a Health Visitor within the Healthy Child Service, you will be working directly with families to help support the social, emotional, physical and intellectual wellbeing of children, from pregnancy to 5 years. Working in partnership with the Health & Childrens Centre Manager you will support in leading the delivery of the Healthy Child Programme 0-5 (HCP) helping plan and review service delivery to ensure it is high quality and delivers outcomes for children.
