The community nursing team across Peterborough is currently offering opportunities for full time hours (37.5 per week) working within CPFT. We provide a service 7 days a week, working closely with other community services and Primary Care Networks to provide high standard holistic care to the population of Cambridgeshire You will have the opportunity to develop yourself, with extensive learning and development opportunities., Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence

  • As an Healthcare Assistant you will work closely with the nursing team to follow a plan of care in deliver high quality, best practice care to adults with a range of complex health needs within their own homes. We are looking for caring, responsive and diverse people who share our passion to deliver our service. We are looking for someone with, + To undertake patients prescribed treatment/care plans, i.e., venepuncture, insulin administration and wound care, and report findings to a qualified professional in line with competency framework. + Communicate with patients/carers by exchanging factual information, reassurance, tact and empathy. + Establish and maintain appropriate working relationships with colleagues, patients, carers and other health care professionals. + Work under guidance on specified tasks, taking direction from registered and non-registered staff as appropriate, with regular clinical support and supervision by a qualified practitioner. + Promote patient independence and self-management Provide and maintain a safe, comfortable and dignified environment for all patients/clients and carers To practice the principles and philosophy of individualised patient care in accordance with Trust Policy, procedures and regulatory body's guidelines. + To maintain appropriate and timely documentation of each patient contact (ensuring these are recorded on SystmOne within 24hours). + Plan and prioritise own visits to delegated patients, (and ensure these are documented on SystmOne) .

    + Experience of working in the community. + Experience of working within health or social care. + Experience of caring for and supporting adults and older people + Demonstrate a commitment to high quality care & service provision. + Good communication skills. + The ability to work flexibly and as part of a team, as well as individually.
