Full Job Description

IST complete nursing assessments and treatment for people with learning disabilities who have behaviours which challenge and mental health needs. IST provides an alternative for service users to hospital admission. The key function of IST is to gatekeep the specialist beds in Dove ward, Kingfisher Court.

IST are a function of the Specialist Learning Disability Service (SLDS). IST comprises of support workers, band 5 nurses, band 6 nurses and a band 7 nurse leader. SLDS is a multidisciplinary specialist health team for people who have specialist health needs related to learning disabilities. The health professionals within SLDS work cohesively to develop specialist assessments, treatment and interventions tailored to the needs of the service users we work with.

We have a 12 month band 5 secondment for a registered learning disability nurse or registered mental health nurse to join the team. The team is based within the East & North Hertfordshire locality with existing links to other local health and social care services for people with learning disabilities. The post operates for 37.5 hours a week with a flexible shift pattern over 7 days., o

To maintain a personal and professional profile to fulfil NMC Revalidation requirements.

o To maintain and promote practice within the Mental Health Act (1983), (Amended 2007), Mental Capacity Act, 2005 and Hertfordshire Safeguarding Adults frameworks.

To work in close liaison with inpatient services.

Undertake clinical risk assessment and management plans and develop within these, comprehensive multi-agency.

o To provide specialist clinical advice for service users that present with behaviour which challenges using Positive Behavioural Support approaches. This will include direct work with service users and their families and carers, advice to colleagues and other service providers and agencies.

o To manage a caseload and operate as a named health worker to service users.

o To identify and provide training in response to specific service user needs within a variety of settings where the care of the individual is provided.

o To carry out joint assessments with other disciplines/agencies where appropriate.

o To provide a person-centred approach integrated within the care planning approach, which optimises integration into community life.

o To act as a practice supervisor or practice assessor for student nursing colleagues whilst they are on placement within the service.

oTo promote effective lines of communication across MDT and all stakeholders.

The successful applicant will be motivated to increase their nursing knowledge and skills in the field of learning disabilities. In return we will provide the post holder with practice experiences, relevant CPD opportunities and excellent clinical and professional supervision to support their development within the IST nursing team.
