Full Job Description

Do you want to be part of improving physical health outcomes for people with a learning disability and autism? Would you like to be part of a progressive and passionate team working closely with primary care?, We are looking for enthusiastic, proactive and innovative individuals with the ability to think outside the box to join our specialist team of learning disability and autism practitioners and health facilitators. You will need excellent communication and liaison skills in order to engage and collaborate with colleagues, those working in primary care and other stakeholders. The successful candidates will have a strong focus on engagement with patients and their families to enable them to access primary care.

The way we care for people with learning disabilities and autism in Suffolk is changing at NSFT as we have recently benefitted from £1.3 million of investment for our acute services and the role out of a new Learning Disability and Autism Specialist Service. The development is fully driven by the national Transforming Care Agenda.

The Learning Disability and Autism Specialist Service is not only being invested in financially,with a fresh approach to ideas and service delivery being supported by the Trust, we are confident our new model of care is the very best for the people we care for.

Then we have exciting opportunities in our Learning Disability and Autism community services across based in East Suffolk for a Learning Disability and Autism Liaison Practitioners, ideally a Band 6 Registered Learning Disability Nurse, however we would welcome other professions with the relevant experience to apply, who has enthusiasm to drive change and a good knowledge base on the physical health needs of people with a learning disability and autism. There will some flexibility as to base location.
