Full Job Description

The post holder will undertake a case manager role, alongside an assessment and formulation role within the team. The Living Well Service will enable timely access to mental health services when people need it most and provide support for people who might not traditionally meet the threshold or criteria of existing mental health services. The team is multidisciplinary, and our works closely with VCSE services., This post is for a Band 6 Mental Health Nurse. The base of the role will be at Hindley town hall however there will be an expectation to travel across Wigan to other Primary care network areas. There is a total of seven spoke areas you will be expected to travel to. You will need to have experience of triaging and signposting referrals as well as being able to liaise with multitude of agencies and professionals. Experience and knowledge of community services and third sector organisations in Wigan will be beneficial for this role. Wigan Living Well Service will be compassionately led, trauma informed, and recovery focused. Development opportunities are available and personal interests / skills will be supported to develop and grow. Living Well has co-produced key values and principles that all staff and peers share.   • Ability to work 5 shifts per week including. Shifts are generally 9 - 5. Some flexibility might be required to meet service demand.

  • Ability to see people flexibly in the community.
