Full Job Description
The Mental health team's key objectives are to support patients and their carers living with functional and organic mental illness such as depression and dementia, in the community. As a support worker, you will work closely with the care coordinators to implement care plans, manage risk, prevent hospital admissions and work with carers to prevent carer burnout and care placement breakdown., The post holder will work within the Ely Older People's Mental Health Team supporting people who live with severe and enduring mental health and dementia.
The post holder will be required to work in a variety of locations such as nursing / residential homes and the patient's own home.
The post holder will be required to take an active role in implementing care and support plans for people as outlined by the care coordinator in collaboration with both paid and unpaid carers
The post holder will manage delegated tasks for a number of service users, whilst under the supervision of a Band 5 / 6 Mental health nurse/practitioner or above.
