Full Job Description

As a member of the organisation, you will have direct contact with children, and young people (CYP), and their families and will be instrumental in ensuring that plans for individual patients, and information that the organisation wishes to share with its patients, are enabled and enacted. The post holder will have excellent communication skills, to ensure that every contact counts, through a range of media (telephone, face-to-face, IT) and will demonstrate excellent patient care through commitment to follow through of established plans to improve health outcomes.,

  • Main responsibilities will be to support families and other professionals to access and understand the referral process
  • To gather information for families, children and young people to support the clinical team
  • To signpost and ensure access to needs led support and advice

    An exciting opportunity has arisen to help play a crucial role in helping people get the right support, at the right time, and to help manage a range of neurodevelopmental needs. Working closely with partner agencies, including parent carer organisations to ensure that the right information and services are involved to best support the identified needs.
