Full Job Description

The Neighbourhood teams in Leeds Community Healthcare provide care and support to people so that they can stay as well as possible for as long as possible in their own homes. This role is to work within our Triage Hub Call Centre and is predominantly a telephony role. Our Triage Hub is the front line for patients, relatives and third-party providers to contact the Neighbourhood Teams. The Neighbourhood Team Officer (NTO) is there to provide administrative support to the Clinical Triagers by answering phone calls from Neighbourhood Team patients, relatives and third party providers and directing them where appropriate. The NTO will also process referrals other associated tasks via the in-house computer systems., The NTO daily activities will include: Taking patient, relative and third party calls Managing patient waiting lists Processing patient referrals Updating whiteboards Managing Email mailboxes Our Triage Hub is open 7 days a week, 365 days a year from 07:00 22:00. The NTO will work Monday to Sunday 7.5 hours shifts of either 07:00 15:00, 09:00 17:00, 14:00 22:00 as required. The base location for this post is Rutland Lodge Medical Centre (LS7) however the successful candidate must be willing to travel Citywide to support where required.
