Full Job Description

The Barnsley Primary Care Network would like to recruit Qualified Nurse Associates to complement our current cohort within the PCN and the clinical workforce as a whole. These roles will be based at assigned practices within a Neighbourhood of our Network, supported by a Practice Nurse for mentorship. Practice location to be confirmed. The role of the Nursing Associate is to work with the Registered Nurse and other members of the multi professional team, providing and monitoring care and contributing to the ongoing assessment, as agreed with the supervising clinician(s) and in line with the service pathways. They will monitor the condition and health needs of people within their care on a continual basis in partnership with the multi professional team, and will be involved in shaping and supporting patients to manage their clinical care needs and you will be central in ensuring they enjoy healthy fulfilling lives. Location of role will be within a GP Practice in the Barnsley Borough. Main duties of the job The Nurse Associate plays a key role in shaping the deliveryof nursing care for the future. We are looking to recruit motivated andenthusiastic individuals to support our nurses to deliver quality clinicalcare. We expect that you will be NMC Registered, able to workindependently and manage your own workload. You will need excellentcommunication skills, able to meet targets and liaise with patients, primarycare staff and other partners. The PCN is interested in supporting individuals with an interest in the Primary Care environemtn to develop their skills within this setting., Thebelow guidance sets out the tasks and / or responsibilities the individual hasin preventing ill health and promoting health in General Practice. The postholder will be responsible for administering immunisations, using behaviourchange and under a PSD SmokingCessation clinics MaintainingInfection Control Standards HealthPromotion advice to patients Supporting theGeneral Practice Nurse with QOF Flu planning Recall ChildHealth, MaintainingSafety and Quality CheckingEquipment / Emergency Drugs oxygen etc. Use behaviourchange interventions to support health and wellness, (for example, NHS healthchecks) CervicalScreening following the Cervical Screening Training Providingand monitoring care The postholder will they contribute to the ongoing assessment of care and be able torecognise when to refer to Senior Colleagues in practice. Demonstrate anability to undertake baseline physical observations and record resultscorrectly. In addition, be able toidentify anomalies that should be referred to the GPN / ANP or GP Medicalstaff. The postholder will contribute to the ongoing assessment of care, undertaking baselineobservations of Blood pressure, Pulse, Respirations, Weight, Height,Elimination pattern, Eating pattern, Sleep pattern, Glucose monitoring Discuss andplan care under the supervision of registered staff, where applicableescalating concerns or complex cases to the GPN / ANP or GP Demonstrateevidenced based knowledge in the main physical health problems that adultpatients may present with , High blood pressure, Diabetes, Obesity,Epilepsy, Respiratory Conditions, Asthma COPD, Chronic Kidney Disease,Cardiovascular Disease, Poor sleep, Constipation, Foot care and Lack ofpersonal hygiene. Demonstrateevidenced based knowledge in Mental problems / Substance Misuse, which mayaffect physical health. Managesituations with patients this may arise from anxiety / emotions relating topoor mental health and or direct them to GPN / ANP / GP as appropriate. Articulate anunderstanding of MDT in and staff roles / Mental Health, community Nursing 0-19services etc. Assess andmanage risk associated with the working environment ensuring appropriate BHFpolicies and procedures are followed accordingly. Demonstrate anability to use a range of therapeutic techniques when dealing with patientswith challenging and unpredictable needs. Understandsthe concept of verbal and non-verbal communication and be able use the skillsat the desired level. Will be ableto discuss and advise patients, with chronic conditions and carers orrelatives. Willcommunicate with the patient about care planning and risk assess report to GPNas appropriate. Communicatecomplex and sensitive information verbally and in writing. Confidentlyand accurately convene patient information in the role of an associate nurse inmeetings Understandsrisk behaviours and risk management. To recordIncidents in line with local policy procedures and standards. To communicaterisk issues as they arise to ensure safe care. The postholder will engage in positive relationships with staff / patients / carers / families and others based on respect and within appropriateboundaries. Act in aprofessional capacity at all times. Activelycontribute to staff Governance meetings. The nursingassociate may wish to lead responsibility in a chronic disease area for example(once more expert) Workingin teams The post holder will support and supervise Trainee Healthcare Assistants withtheir learning and development, also trainee nursing associates, apprentices,health coaches and those new to care roles. The postholder will supervise volunteers If this is required. Improving safety and quality ofcare Nursingassociates role in maintaining and improving care, will be to support BHF withresearch and evaluation, quality improvement strategies. The Nursingassociate will participate in audit, health and safety activities, as discussedwith Head of Nursing Nursingassociate will escalate any risks or safeguarding concerns to the GPN andfollow the policy for safeguarding. The postholder will escalate any specific risk / concerns to the staff on duty and actaccording to local policy Contributing to integrated care Post holderwill refer to agencies as necessary Community Nursing / SocialPrescribing, receive referrals to or from, or co-ordinate care with the GP The postsresponsibilities are to refer for blood tests as delegated by GPN - Tasksrelating to these teams or agencies / wound care / phlebotomy etc. Responsibilitiesare the safe transfer of care between professionals, settings or services usingSystmOne The postholder may work across the PCN in other roles if employed such as Health Coach Responsibility for InformationResources Record careundertaken in the SystmOne notes. Maintain notesthat are accurate. Ensure patientrecords comply with issues relating to confidentiality and safe storage. Be able to usethe email system in line with BHF local policy and procedure. Zero Tolerance Ensure thatactive de-escalation and diffusion techniques are used to minimise risk ofphysical aggression, during which the safety of patients, self and othermembers of the team are considered a priority. Emergencies Respond toemergency situations as they occur and be prepared to act as support wherenecessary. Carry outphysical / psychological care for service users , enhanced observation andfollow the required reporting protocols.
