Full Job Description

As a Nurse Care Manager you will be central to a network of partners working across health and social care in Leeds who are committed to increasing integration for better services supporting people with learning disabilities.

You must be registered with the Nursing and Midwifery Council as a Learning Disability Nurse on commencement in post.,

  • knowledge and understanding of current learning disability practice and the role of a multi-disciplinary team, including statutory and voluntary agencies
  • skills in effective assessment, planning, intervention and evaluation of work in supporting people with learning disabilities
  • ability to build good working relationships with professional colleagues and partners and engage positively with individuals, families and carers
  • experience of using a person-centred, holistic approach and creativity in problem solving and reflective practice techniques
