Full Job Description

Exciting opportunity to work within the WSH Outpatient Diabetes Team supporting patients with diabetes who are under Consultant Led Care. This post would include Antenatal and insulin pump diabetes care. Requires a high level of self-direction and autonomy to manage a specific caseload.

Candidate will run Nurse Led Clinics, insulin start groups and provide structured education for patients with diabetes. Follow up of these patients will involve email, telephone and teams contact in between Consultant Clinic reviews. Ability to work autonomously with an organised approach is essential along with effective communication skills.
