Full Job Description

The post holder will provide a comprehensive service to the Trust with a focus to facilitate effective and timely discharge of patients no longer requiring acute care. The post holder will provide a single point of contact for the management of bed availability and allocation within ward. The role requires a consistent and coordinated approach to the operational management of patient flow, with an emphasis on maintaining an on-going and 'real time' understanding of the ward's current and predicted capacity position and the timely and well planned discharge process., To support the ward manager and ward co-ordinator in maintaining patient flow To manage the maintenance of e-track to ensure it reflects the current discharge plans and the ward bed availability Liaise with the duty manager when a bed is available. Attend Whiteboard rounds, identify actions required and actively manage agreed discharge plans for patients with predicted discharge dates within the next 3 days. Maintain whiteboards and e-track information to ensure it is up to date. Arrange or where necessary photocopy notes for patient transfer. Assist in the completion of discharge check lists to ensure accurate discharge planning is evident. Chase doctors for completion of discharge letter and deliver TTOs if necessary. Ensure diagnostic requests are expedited promptly and escalate any delays to the appropriate person.
