Full Job Description
The post holder will assist in the management of the delivery of care to patients within the Bradford and Airedale Regions. Clinics are held at the St Luke's site and at Skipton and Airedale. Educational visits are performed in the patient's home, or for the acute patients, as an inpatient at the Bradford Royal Infirmary. To meet the needs of the service the post holder will be expected to travel between sites and undertake work in the community. As well as face to face consultations we offer telephone advice to patients and healthcare professionals, and also e consultations via SystmOne for primary care.
The post holder will be expected to work in conjunction with the multidisciplinary team/ external agencies responsible for care provision to further develop this service in line with local and National guidelines, collaborating in service development and sharing best practice, 1. Assists the Senior Pre- Dialysis Nurse in the organisation and management of all patient care maintaining high standards of care.
2. Develops leadership skills both professionally and managerial.
3. Assists in ensuring effective communication with all levels of staff and the whole
Multi-disciplinary team.
4. Assists in assessing the overall resource requirements for the specialist area and manages this effectively.
5. Assists in implementing and maintaining policies, ensuring staff compliance.
6. Initiates and develops new ideas for the improvement of services to patients in consultation with the line manager and other staff. This includes involvement in implementing Renal NSF, and achieving Local /National standards and guidelines including NHS England service specifications.
7. Co-operates and is flexible in working pattern to meet service / patient needs.
8. Assists in considering skill mix to develop a workforce capable of meeting the needs of the patient.
9. In the ward managers absence, investigate and report on complaints from patients / carers / relatives and staff and reports to either the ward manager or Patient Services Manager.
10. Investigates and reports incidents and untoward occurrences and reports to line manager/ Matron.
11. To promote learning for all staff through risk incident reporting.
12. In the absence of the ward manager attends regular meetings with the Patient Services Manager/Matron . Provides effective channels of communication.
13. In the absence of the ward manager Team Briefs all staff reportable to the ward manager.
14. Assists in fostering an environment that empowers staff and is conducive to change and development.
15. Maintains a safe environment in the renal unit for patients / staff and visitors. Follows all Health and Safety policies.
16. Co-operates in the trials of equipment and resources
17. Ensures the safe custody and storage of drugs held within the area and the practice of a regular checking system.
18. Is involved in the recruitment, selection and retention of staff.
19. Collaborates with professions allied to health.
20. Attends and assists in ward meetings.
PROFESSIONAL
1. Follows the NMC Codes of Conduct/Practice.
2. Develop research skills in self and others that promotes evidence-based practice.
3. Maintains an environment conducive to learning for all staff in training is involved in Educational Audit and liases with Link Lecturer and the Practice/Professional Development Nurse for the unit.
4. Implements and develops systems of care deliver in conjunction with the line
manager
5. Exercise professional accountability for their practice, ensuring that any expansion of the role is achieved within the scope of professional practice.
6. To be involved in developments relating to clinical governance.
CLINICAL
1. Maintaining clinical credibility through regular clinical practice updates.
Participate in the clinical practice assessments of staff including Cannulation, phlebotomy
and administration of medications .
2. Keeps up to date with innovations in dialysis and nephrology nursing by attending educational,
events, study days, conferences and journal reading.
3. Working closely with the Practice/Professional Development Nurse for the unit, identifying learning needs of staff to develop safe and competent practitioners.
4. Leading staff in co-ordinating and managing care of patients.
STAFF/SELF DEVELOPMENT
1. Agrees Personal Development Plan.
2. Assist with the carrying out of appraisals on all members of staff, where possible.
3. Assists with the maintenance of training records.
4. Ensures the environment and resources available for staff are conducive to job satisfaction and high morale.
5. Attend mandatory training sessions as per Trust Policy and NMC regulations
6. Becomes familiar with the role of the lead nurse and other ward managers within the unit .
7. Manage accurate staff records, including sickness and absence, performance issues etc.
8. Assist the ward manager in providing an Induction Programme for all staff, ensuring that there are appropriate mentors and preceptors for all qualified and unqualified staff.
9. Be involved with Clinical Supervision.
10. Assists the ward managers in ensuring standards are audited as per Essence of Care and action plans are instigated before reporting to the Patient Services Manager/Matron.
TECHNOLOGY
1. Assists in ensuring that all staff are trained in the correct usage of equipment.
2. Assists with maintaining accurate training records
3. Keep accurate records of equipment on loan/hire.
4. Develop information technology skills.
Professional Registration
Where the post holder holds a professional qualification they will be required to;
· Maintain their own professional status and registration where appropriate
· Work within the professional regulations set by the relevant Professional Body. Currently in clinical job descriptions only I can't see any reason why

Infection Prevention and Control

This includes a requirement to maintain a safe, clean and tidy work environment and to complete mandatory Infection Prevention and Control Training as provided by the Foundation Trust. Communication and Working Relationships
Internal
The jobholder will be expected to communicate with various wards, departments, Consultants and their secretaries and other Allied Healthcare Professionals.
External
The jobholder will be expected to communicate with Primary Care Trusts, their General Practices, District Nurses, Nursing and Residential Care Homes and those services, which come under the NHS. The jobholder is also expected to communicate with other groups that fall outside the NHS, such as Social Services.
An opportunity has arisen for an enthusiastic, motivated and organised individual with a background in renal nursing to join the predialysis team in Bradford. The team is involved in providing education and supportive care to patients with advanced chronic kidney disease including anaemia management, referral for dialysis access and coordination of care for those choosing not to have dialysis. It is expected that the post holder will be able to work independently alongside the senior predialysis sister and other members of the multidisciplinary team. Day to day work will be varied including home visits, clinics, ward environment and office based. The area we cover is Bradford and Airedale, the post holder will need a full driving licence and have access to a car The post is: Part time.
Working Monday to Friday
Applicants should be a Registered Nurse with significant post registration experience including experience of working in the renal setting for a minimum of 2 years.
