Full Job Description

Adult Physical Health services are divided into 4 categories Anticipatory, Planned and Unplanned Care and, Specialist services. All services within aim on improving outcomes for people and maximising access to services through integrated working.

Working alongside the District Nursing Service with an Anticipatory Care focus the aim being to reduce pressure on planned and unplanned care services. Supporting, teaching, and providing advice and support following best practice guidance is key to the role. To optimise patient care and ensure that where possible that patient independence is preserved and maintained.

The Community Teams aim to maintain people within their own homes/communities wherever possible and facilitate earlier discharge from hospital through delivering a more co-ordinated approach to care.

The Community Teams work closely with their colleagues in General Practice and the Third Sector organisations. Central to the philosophy and approach the Community Teams are placing the individual at the centre and proactively maximising opportunities for people to have choice, personalisation and self-care., Supporting, teaching, and providing advice and support following best practice guidance is key to the role. To optimise patient care and ensure that where possible that patient independence is preserved and maintained.

Self Management Facilitators within the Community Teams aim to maintain people within their own homes/communities wherever possible and facilitate earlier discharge from hospital through delivering a more co-ordinated approach to care.

The post holder will work within standard operating procedures / pathways to manage their own workload independently but with reference to a supervisor within the clinical area. In doing so, they will support the assessment of patients and implementation of programmes of care ensuring that they practice within their sphere of competence and knowledge. The post holder will work within a range of standard operating procedures and pathways in which they have received training in order to be competent.
