Want to make a difference in Hull? As Hull City Council place major investment into SEND services, an exciting opportunity has arisen to join the SEND Team as a SEND Caseworker with a focus on Post 16 provision for young people with SEND.
By joining our team, you will be making a tangible difference to the lives of people in Hull and working with people who are as passionate about our place and public services as you are.
You will be required to provide proactive casework management, working in a responsive and customer focussed manner.
The SEND Caseworker role is to co-ordinate support for children and young people who have an EHC Plan or who are going through the Education, Health and Care Needs Assessment Process (EHCNA). The role of a SEND Caseworker includes the following -

Co-ordinates the effective and timely delivery of the EHCNA process for children and young people with Special Educational Needs and Disabilities (SEND) across the 0-25 age range
Monitors Annual Review meetings for children and young people with an EHC plan who are educated in settings
Process and complete required actions from Annual Reviews. This includes amending EHC Plans following an Annual Review decision to amend
Co-ordinates person-centred Annual Review meetings for children and young people with an EHC plan and who are educated otherwise than at school or educated within the home
Co-ordinates the educational placement of children with EHCPs including transition into primary, secondary or post-16 provision
Represents the local authority at mediation and through the disagreement resolution service
Works closely with Health and Social Care on placements of children in out of area educational providers
Contributes to the strategic development of SEND Services in Hull
Inform parents/carers of the process and eligibility regarding personal budgets for provision of SEND support. Ensuring these are regularly reviewed to reflect the changing needs of the child or young person.
