Full Job Description

Manage a designated group of patients for the Integrated Care Team. Provide nursing assessments, diagnoses, treatments, and advice, and conduct general assessments for the team. To work independently under the guidance of service lead and matron and actively participate in the professional development of self and others. Ensure the implementation and evaluation of care in conjunction with other agencies incorporating statutory and voluntary services. Manage a team with varying skill mix to support a Home First approach To provide an in and out of hours service at weekends, evenings and bank holidays Main duties of the job The role will be supported by our clinical team leads. You will gain the opportunity to lead and manage within our integrated care hub whilst also extending your scope of practice in areas such as the admission avoidance, supported discharge and working closely with allied health professionals. Applications will be welcomed from candidates who ideally have some community nursing experience and those who have the drive to succeed. Open mindedness around practice is required as you will be working with a range of health care professionals widening your skills beyond the realms of traditional community practice. As a senior nurse you will progress to a level whereby you will feel confident in independently leading a team across a variety of shifts patterns. You will respond to situations appropriately as they arise with support from your senior and management colleagues.   • Qualified Registered Nurse with NMC PIN
  • Full UK driving licence and access to a car
  • Understanding of the role of community nursing
  • Desirable
  • Good IT skills
  • Experience of working with patients with frailty
  • Knowledge of Home First / D2A
