Full Job Description

The team provides rehabilitation to patients in their own home & residential care. The service provides specialist community therapy assessment & treatment to improve health & independence. In addition, the service offers rehabilitation to enable earlier discharge from hospital or to prevent hospital admission.

You will work as part of a multi-disciplinary team, following initial assessment by a registered health care professional you will exercise initiative & judgement in care planning, goal setting, delivery, implementation & evaluation of care, working collaboratively & in partnership with others

You will contribute to enabling service users to reach their potential & develop knowledge to promote holistic care, including supporting patients who have longer term rehabilitation needs.

Hours of work will include weekends and bank holidays.

Work in close liaison with the wider MDT

Provide high standards of safe & compassionate care to people that maximises their rehabilitation potential

Participate in training, education, & clinical supervision to develop skills & knowledge & apply continuous reflection on own practice

Assist with student placements & help develop others

Possess high levels of communication & excellent interpersonal skills & time management skills

Be committed to teamwork & delivering high quality patient care

Exercise a degree of independence, under supervision, using your initiative & judgment when delivering tasks

Develop & maintain meaningful therapeutic relationships with people, ensuring the care experienced is positive & service user safety is maintained

Take part in service developments & show flexibility when adapting to & supporting change

We are looking to appoint a motivated & forward-thinking Senior Therapy Assistant to join the Neighbourhood Rehabilitation Service (NRS) which encompasses the Intermediate Care team and Long-Term Condition Pathway, working across Barnsley Community.
