The role of Telecare Coordinator is to support the implementation of Telecare and other assistive technology across the city of Hull by undertaking assessments that informs care and support planning to integrate the use of assistive technology / Telecare to prevent or delay the need for long term care support packages. The role enables the opportunity to work with partner agencies to assist in the prevention of unnecessary hospital admission and facilitation of discharge from acute care through the mainstream use of Telecare in service provision. Additionally, to work in partnership with Health and Housing to actively promote the use of Telecare as a primary support. It is essential that the candidate can work collaboratively with other professionals and partners to ensure the best outcomes for the person. The successful candidate will have the following:

  • Up-to-date knowledge and keen interest in assistive technology, have well-developed communication skills and experience of assessing.
  • Knowledge of relevant legislation including the Care Act and Mental Capacity Act.
  • A passion to work with people to enable them to live their life and not receive a service
