Full Job Description

Would you like a role working in an adult community setting offering variety, #no2daysthesame, working with a range of professionals in dynamic, innovative, and supportive teams? We have an exciting opportunity to work in our Neighbourhood teams in Leeds, alongside physiotherapists and occupational therapists as a Band 4 Therapy Assistant Practitioner., Key responsibilities are delivering holistic rehabilitation, which includes provision of equipment, teaching and progressing exercise programmes and signposting to community resources to encourage social engagement. You will also be supporting progression of ability to self-manage activities of daily living, including meal preparation, personal care and medications. You will have opportunities to assist with student placements, help develop others, contribute to service improvement projects, and enhance your own skills, including an option to work towards applying for a degree apprenticeship. The service is provided over core hours of 8am 6pm across 7 days. Weekend and unsocial hours attract enhanced pay. We support a positive work life balance so when aligning you to a team we aim to consider your personal preference and consider any caring responsibilities that you may have. At times you may be required to support other teams to support workflow and meet service demands.

We are looking for someone with previous experience of health and social care, with excellent communication and organisational skills, looking to further enhance their skills in person centred rehabilitation.
