We have an exciting job opportunity for a Patient coordinator to join our ever-growing team, where you will be given the scope to deliver and add real value to the organisation. You will be dealing with referrals for the National Diabetes Prevention Programme (NDPP), providing inbound and outbound telephone and administrative support. Our health & wellbeing's experienced team is dedicated to making the UK's local communities healthier. We help people to understand and take control of their own health through sustained behaviour change, transforming lives for the better through weight management, quit smoking services, diabetes services, cardiovascular disease prevention and health coaching & social prescribing. - Contact service users following receipt of their referral, you will be promoting the programme and offering assistance and resources, all while providing outstanding customer service.
- Complete initial assessment during booking calls and triage for further coaching calls where appropriate.
- Self-manage reporting waiting lists in line with sessions available to offer.
- Handle multiple inbound call queries and register service users on to our programme, being helpful and assisting patients on their journey with us.
- Manage and coordinate bookings of one-to-one and group appointments for service users within the programme.
Previous experience within customer care, ideally in complaints or hospitality - Excellent IT Skills - proficiency in Microsoft packages, mainly Excel and Outlook
- Excellent written and verbal communication skills.
- An ability to deal with challenging situations with customers, and provide effective solutions.

Full time, Mon - Fri 9 - 5 with 2 late shifts, (11 - 7 Mon and Wed) and 1 x Saturday shift, 9 - 5 per month £23,400 per annum, plus bonus and excellent benefits,
