You will support young people and families by ensuring their transition occurs in a cohesive and a coordinated manner and there is effective liaison with central transition teams in adult services to facilitate transition and problem solve barriers jointly. Develop and provide suitable transition and adolescent health training packages for various staff groups with the Trust and ensure all specialities have transitional pathways which are developed jointly with adult services.
