Full Job Description

The purpose of the Ward Discharge Co-ordinator role is to ensure that patients have a planned, timely discharge and that the patient's pathway is effectively managed, ensuring patients have a positive experience of the hospital discharge process. The Ward Discharge Co-ordinator works closely with the clinicians, ward clerks and the Integrated Discharge Team to ensure that patients are proactively managed through their pathway ensuring key elements of the patient's pathway such as investigations, medication and transport are requested and actioned in a timely way.

The role also involves ensuring that the Trust's electronic systems are kept up to date and that key elements of the patient's pathway are recorded on the relevant systems. The Ward Discharge Co-ordinators are responsible for liaising with Ward Managers to ensure that any potential delays to the discharge process are identified and actioned appropriately., Discharge planning and supporting ward staff in providing efficient, safe discharge for our patients.

You will work closely with all members of the multi-disciplinary team including medical and nursing staff, ward clerks, the Integrated Discharge Team to ensure that patients are proactively managed throughout their pathway towards their estimated date of discharge.

You will co-ordinate key elements of the patient's journey such as investigations, medication and transport to facilitate a safe and timely discharge.

A flexible and pro-active approach is essential for this post, as are excellent communication and team working skills.

IT skills are essential as the post also involves accurately updating of the Trust's electronic information systems.
