Full Job Description

We are seeking to appoint a GPs with an interest in Frailty to support the delivery of the various developing Frailty Services, including but not limited to

  • The York Frailty Crisis Response Hub
  • York Frailty Outreach Service
  • The York Integrated Community Team MDTs
  • Supporting Virtual Frailty Ward patients remotely.
  • You will need to hold the relevant professional registration, GMC and have experience of Triage Telephone Assessment. Experience of working in Geriatric Medicine is desirable. Any successful candidates would join our growing team of GPwERs in Frailty and be supported to obtain their Diploma in Geriatric medicine as well as join our regular education sessions. The Frailty Hub has three different arms, the team case manages 3000 of the most frail and vulnerable patients in York and supports with a Frailty Crisis response as well as pulling frail patients out of hospital whenever possible. This is predominantly a face-to-face role based at Acomb Garth Community Centre between the hours of 8.30am and 5pm. You will be required to work 2-4 sessions per week., You will undertake a variety of duties, mostly offering support and advice to System providers regarding vulnerable patients in Community as part of an MDT, undertake best interest discussions with patients and family members and help co-ordinate a response to support people in need to stay home whenever possible. You may be needed to visit a complex frail patient at home or undertake telephone consultations. Make professional, autonomous decisions in relation to presenting problems. Assessing the health care needs of complex patients with undifferentiated and undiagnosed problems, highlighting which services would support them best to stay in their home setting and independent whenever possible. Cover the clinical management of patients as appropriate including initiating investigations, reviewing results, making referrals to other providers/services as appropriate. Record clear and contemporaneous computerised consultation notes to agreed standards. Compile and issue computer-generated acute and repeat prescriptions. Promote the application of standards and codes of Practice set by the Royal College of General Practice, General Medical Council, Department of Health, British Geriatrics Society and local NHS trust. Participate in national frailty training and maintain high volume of CPD in Frailty related conditions with the aim of obtaining the Diploma in Geriatric Medicine.
