Full Job Description

We're looking for an experienced registered nurse to join our Admiral Nurse service, which delivers dementia care for people in Milton Keynes. The Admiral Nurse service works in partnership with Dementia UK to improve the experience of carers and families affected by dementia with complex needs, enabling people with dementia to stay in their homes for longer by supporting the carers to become more resilient and empowered. Reporting to our Admiral Nurse Clinical Lead, you'll manage your own case load, run groups, provide training sessions and act as a role model to promote outstanding dementia care. You'll be able to evidence your excellent knowledge of dementia and older people through clinical post-graduate training, education and experience. This role is based at Civic in the city centre and you will be working with families in their communities. Interviews for this role will be held on 19 November.Main Responsibility

  • Provide practical and emotional support/advice to carers, empowering them to work with their loved ones to create their own care plans which reduce risks for all, promoting self-care and resilience.
  • Manage a caseload in the community, completing triages, holistic assessments of need and risk management.
  • Responding to these needs by providing biopsychosocial interventions, education, and advice.
  • Working collaboratively with health, social care and voluntary sector services or referring on to services and signposting as appropriate.
  • Attend and actively participate in practice development, regular clinical supervision, reflective practice sessions and continuing professional development, ensuring up-to-date knowledge of evidence-based recommendations on supporting people with dementia and their carers/families.
  • Facilitate and support the development and delivery of innovative dementia training and education to professionals, to improve competence and confidence, in line with best practice and guidance.
  • Facilitation and delivery of support/educational groups and physical activities.

    Must hold a Registered Nurse (RN/RNMH/RNLD) qualification and maintain professional registration in line with NMC guidance, including re-validation and a full DBS check.
  • Dedicated to improving the lives of those living with dementia and their carers, acting as an advocate to promote individuals rights and best interest through role modelling, best practice and evidencing up to date knowledge of interventions, relevant policies and safeguarding.
  • Excellent communication skills and the ability to manage conflict and emotive situations in a calm and proactive manner.
  • Will have ability to use IT systems and software effectively to accurately record and communicate.
  • Flexible - you might be asked to work in another place or outside of office hours to meet carers needs.
  • Must be able to get about. For this role you are required to have a full UK driving licence and provide evidence of your vehicle insurance with business cover and use of a vehicle. If you are successful, you will be required to supply evidence of your licence prior to starting the role.
  • Should you be offered the role, as part of the pre-employment checks an Enhanced DBS will be actioned for you (unless you are on the DBS Update Service).

  • Salary £40,221 - £44,428 - this salary will be pro-rota based on the hours 22.5
