Full Job Description

Rayleigh and District Primary Care Network are looking for a dedicated and experienced advanced nurse practitioner to join our team on a full time basis and contribute to our mission of delivering exceptional healthcare services to our community. Based in our Rayleigh practice, you will play a pivotal role in delivering advanced clinical care to our patients. You will work autonomously, providing assessment, diagnosis, treatment, and management of a wide range of conditions, both acute and chronic. Due to home visits the applicants will need to hold a valid full drivers licence.,

  • Conduct comprehensive patient assessments, including history taking, physical examinations, and ordering diagnostic tests.
  • Diagnose and manage both acute and chronic conditions, making appropriate referrals where necessary.
  • Prescribe medications and treatments in accordance with current guidelines and best practices.
  • Provide expert advice and education to patients and their families regarding treatment options, preventative care, and self-management.
  • Collaborate with the multidisciplinary team to develop and implement care plans that meet the individual needs of patients.
  • Participate in the development of clinical protocols and pathways to improve service delivery.
  • Lead on quality improvement initiatives within the PCN.
  • Engage in professional development and training to maintain and enhance your clinical skills and knowledge.
