Full Job Description

Join our friendly team as a Part-Time Advanced Nurse Practitioner at Mountain View Health Centre.

The hours are Monday to Friday 30 hours per week, 6 hours per day

The Advanced Clinical Practitioner (ACP) plays a pivotal role in enhancing the level of care provided to patients through advanced practice interventions. This role involves autonomously managing complete episodes of clinical care across all age groups and all demographics, encompassing a wide range of clinical conditions, diagnosing, treating, and referring patients as appropriate. The ACP operates within a multi-disciplinary team and is instrumental in mentoring and supporting both nursing staff and trainee practitioners.

Key responsibilities include the application of advanced clinical skills to assess, diagnose, treat, prescribe and advise patients across a variety of settings including face to face, telephone and e-consultations, ensuring the use of evidence-based practice. The ACP will also lead in the development of new services and pathways to improve the quality of care and patient outcomes. This role requires dynamic clinical leadership and activities to promote a culture of learning and continuous improvement within the practice.

Main duties of the job

Clinical Responsibilities:

Patient Care: Deliver advanced and complex clinical care

Autonomous Practice: Work independently within the scope of practice to manage complete and undifferentiated clinical episodes

Case Management: Manage patient cases from initial assessment to conclusion.

Prescribing: Utilise prescriptive authority responsibly

Leadership and Guidance:

Mentorship: Provide mentorship and supervision to nursing staff

Service Development: Lead the development and implementation of new clinical services

Professional Development: Engage in continuous professional development

Research and Quality Improvement:

Research Participation: Participate in clinical audits and research activities

Quality Assurance: Implement quality improvement initiatives

Educational Activities:

Training Provision: Deliver training sessions to healthcare staff

CPD Activities

Administrative and Organisational Duties:

Record Keeping: Maintain comprehensive records using the practice software.

Meetings: Attend and contribute to team meetings

Mountain View Health Centre is a GP Practice situated in the Mayhill area which is a deprived area of Swansea.

We are a single site practice with over 6000 patients registered at our practice. We are housed in a purpose built modern GP surgery, expanding our capacity to provide high-quality care to our patient community.
