Full Job Description

You will be part of the development of new ways of working within the community. You will be given the skills and training to undertake a variety of tasks to a care for a delegated cohort of patients to support their health and wellbeing.

This is an exciting time to be joining the community nursing teams as we have been given an opportunity to truly challenge and change how we deliver community services to housebound patients to improve patient experience and outcomes as well as your working lives.

You will be part of the development of new ways of working within the community.
