We have agreat opportunity for aCare Navigatorto work within the Learning Disability and Autism Service.

The Care Navigator role is specifically to improvehealth outcomes for adults with a Learning Disability and/or Autism and their families. The Care Navigator is a member of the Administrative team supporting the Learning Disability Community Teams. This involves working closely with all staff/professionals in the Learning Disability Service, Voluntary and Local Authority Services.

The Care Navigator Teamwill support with promoting the Learning Disability and Autism Services at stakeholder events across Leicester, Leicestershire and Rutland.

You must have access to your own transport or suitable alternative. Dependence on public transport is not suitable for this role.

This postis a 12-month fixed term contracts or secondment.Approval from your current line manager is required prior to applying for secondment., The Care Navigator role is specifically;

- To improve outcomes for adults with a learning disability and their carers.

- Will involveworking closely with the key stakeholders to co-ordinate and administer the processes of multidisciplinary teams within a defined geographical area.

- Seeking out cases where healthcare arrangements have become unnecessarily complicated, reduce complications, improve coordination, and actively engage all relevant parties.

-The Care Navigator will focus on health needs specifically relating to the person when they have additional needs or may have social care involvement.

- To collate service user feed back during and following discharge from care.

- To collate and report regular data information, support audits and attend the Patient Tracking List meetings

- Proactively useelectronic record systems to identify services involved and liaison with relevant professionals to provide timely joined up information to support service users and families

- The Care Navigator will arrange and facilitate where appropriate multi agency discussions (internal and external to the organisation)., patterns in line with our flexible working policy. For all substantive roles, new staff (excluding medical staff) are appointed subject to a 6-month probationary period (see Probation Policy). All jobs will require permission to work in the UK.
