Full Job Description

You will be based at L11 Redlands Road, working between the hours 8am - 8pm (8-4 or 12-8) to cover the Community Children's Nursing service Monday - Friday and 9-5pm on Bank Holidays. You will also form part of a 24 hour rota if a child is at end of life and you will be part of a Rapid Response rota with the Police, visiting homes where a CYP has died unexpectedly and offering oversight from a nursing perspective. This is a varied role where you will hold a caseload of children and young people (CYP) with a wide range of health conditions in Reading, West Berkshire and Wokingham, acting as a keyworker for CYP and families, being the link between hospital and the community.,

  • Teaching parents/carers on clinical procedures using competency-based training.
  • Signposting to other appropriate sources of support and liaising with other professionals involved as required/appropriate.
  • Providing excellent standards of clinical nursing care, support and advice to children and their families, including regular reviews, visits and attending clinic appointments as needed.
  • Being part of multidisciplinary meetings with allied professionals, including discharge planning meetings, safeguarding meetings, team meetings etc.
  • Central line maintenance and all associated cares, including taking bloods as required.
  • Administering medications such as IM injections and intravenous antibiotics.
  • Raising concerns appropriately and being involved with safeguarding processes and attending any Child in Need / Child Protection meetings as required.
  • Contributing to audits and to service improvement.

    For this role you will need to have good clinical and organisational skills and the ability to work independently and autonomously, whilst also remaining part of the wider team. Effective communication with the multi-disciplinary team is vital with a good knowledge of safeguarding.
