Full Job Description

An exciting opportunity has arisen within the Fracture Liaison Service for a band 7 Fracture Liaison Service CNS role. The Fracture Liaison Service works with patients over the age of 50 sustaining a fragility fracture to identify those with Osteoporosis / at a high risk of further fractures and placing these patients on a personalised treatment pathway. We work alongside multiple specialist teams including Rheumatology, Trauma and Orthopaedics, Radiology and the inpatient wards. The successful candidate would work closely with patients, primary care teams and specialist services within the Berkshire area to promote bone health and Osteoporosis knowledge.,

  • Works across acute trust and CCG, providing specialist clinical services to a defined patient/group.
  • Works strategically to influence the development of services both locally and nationally.
  • Participates in local clinical audits and national collaboration for FLS.
  • Provides specialist bone health/Osteoporosis education and training for colleagues, students and patients.
  • Works with the lead clinicians, care group managers and directors, to set and deliver the strategic vision of the services.
  • Implements policy/national guidance to support the development of the service.
  • Display excellent communication and leadership skills.
  • The postholder would ideally have good Orthopaedic and/or Rheumatology experience.

    We are looking for a colleague who enjoys working as part of a small but well-established team and has experience of autonomous working. A passion for working in preventative health care and supporting patients to remain fit and active as they mature is a must. Experience working in Trauma and Orthopaedics or Rheumatology would be strongly desirable. In exchange we can offer a flexible working pattern, the ability to work remotely once competent and a supportive wider team.,
  • Registered Nurse with significant experience.
  • Educated to a minimum of 1st degree level or equivalent experience / able to demonstrate evidence that is working at degree level
  • Evidence of recent clinical practice in specialist area/ evidence of recent appropriate experience.
  • Evidence of Leadership, management and specialty development
  • Experience in Research and Audits
  • Desirable criteria
  • Nurse Prescribing Course/ Willingness to undertake
  • Demonstrates a desire to explore innovative ways of delivering services, Experience in teaching, assessing and coaching
  • Recent clinical experience in related client group
  • Demonstrates knowledge of quality improvement and evidence of working towards improving KPI's
  • Excellent interpersonal skills with particular ability to liaise effectively with multidisciplinary team members
  • Experience of data collection for audit, research and quality improvement
  • Desirable criteria
  • Experience in presenting to clinicians
  • Experience in working both as part of a team and autonomously, You must have appropriate UK professional registration.
