Full Job Description
We are looking for a dynamic and motivated band 6 to join our busy Vascular Access team. This post will be a development post where you will be trained in the insertion of vascular access devices; these will include peripheral cannulas, Midlines, PICC's, Femoral catheters and Tunnelled Central lines. This post holder will contribute to the day to day clinical care and assessment of patients requiring vascular access and manage their own clinical work load. The post holder will also be required to provide education to all staff at a corporate and multi disciplinary level and support clinical teams on the ward to manage the inserted devices on a day to day basis. The post holder will keep accurate documentation to reflect the insertion, care and follow up that they have provided. This post is based over all four Oxford University hospital sites and the post holder will be required to travel between them This post will progress to a band 7 on completion of a professional portfolio. Proven band 6 experience is essential Assessment of patients requiring a vascular access device Ultrasound guided insertion of vascular access devices - Peripheral cannulas, Midlines, PICC's, Femoral catheters and tunneled central access devices Education on corporate study days and continued education for ward based staff Working under OUH PGD guidelines IRMER registered once in post Continued professional development Oxford University Hospitals NHS Foundation Trustis one of the largest NHS teaching trusts in the country. Watch how we set out to deliver compassionate excellence via theOUH YouTube channel. The trainee SSNP will work as part of the autonomous specialist nursing team. The role holder will be responsible for the clinical examination and assessment of patients in order to select and insert appropriate venous access devices for patients requiring intermediate and long term intravenous therapies throughout the OUH Trust. The role includes follow up of line care, trouble-shooting and education for all members of the MDT. 'A highly experienced, knowledgeable and educated member of the team who is able to work autonomously making high level decisions to treat health needs.' Advanced level practice encompasses aspects of education, research and management but is firmly grounded in direct care provision or clinical work with patients' families and populations. (CNO 2010) The Senior Specialist Nurse Practitionerwill have completed and maintained a professional portfolio relevant to Advanced Vascular Access. They will be competent to work autonomously in all aspects of Vascular Access covered by the Vascular Access Service. The trainee SSNP is a developmental post that is working towards becoming a Senior Specialist Nurse Practitioner with the key result areas outlined in the band 7 job description. The role is supervised and clearly linked to competencies that require completion within a pre-arranged and designated time frame of 12-18 months. The focus will be on developing autonomous practice. Demonstrated proficiency will advance this role from band 6 (trainee) to band 7 as a Senior Specialist Nurse Practitioner. The SSNP will be able to; Insert Vascular Access Devices within the practitioners competence
- Work at an advanced level practice autonomously as well as within a multi-disciplinary team and are self directed.
- Work clinically to optimize health and wellbeing, demonstrate autonomous decision making related to comprehensive history taking and physical assessment of patients.
- Work at an advanced level drawing on a diverse range of knowledge in complex decision making to determine evidence-based therapeutic interventions.
- Plan and manage complete episodes of care, working in partnership with others, delegating and referring as appropriate to optimize health outcomes and resource use, providing direct support to patients and clients.
- Use professional judgement in managing complete and unpredictable care events.
- Draw upon an appropriate range of multi-agency and inter-professional resources in their practice.
- Appropriately define boundaries in practice.
- Promote a high standard of advanced specialist nursing by initiating and co-ordinating the assessment, planning, delivery and evaluation of the holistic needs of patients/families through evidence based practice following agreed policies, protocols and guidelines.
- Practice a high standard of communication including highly sensitive and complex information to develop and sustain partnership working with individuals, groups, communities and agencies. Utilise evidence based information through a variety of communication media.
- Maintain adequate patient documentation to NMC requirements for all patients seen and advice given in any practice setting and contribute to clinical activity/data collection as required.
