Community Health Care Support Worker

An exciting opportunity has arisen for a permanent Band 3 (18.75hrs per week), Community Healthcare Support Worker, to work in the 2T's to support the Community Diabetes Specialist Nurse as part of the Chronic Disease Management team.

The team is friendly, motivated to learn, and there are opportunities to further your career development. The post holder will need the ability to travel between sites in a timely manner, Main duties involve supporting the Community Specialist Diabetes Nurse in the management of patients in their own homes with complex Diabetes needs.

The post holder will need excellent communication skills and be motivated to learn new skills

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
