Full Job Description

1. To be responsible for the case management of a caseload of clients, including coordinating their care
2. To ensure provision of high quality and evidence-based care
3. To assess, plan, implement and review care in line with NICE guidelines and the Care Act
4. To offer and conduct carer assessments in accordance with the Care Act 2014 or to link with relevant other services providing this assessment
5. To ensure service users and their carers take as full a part as possible in case discussions, care planning and implementation
6. To offer psycho-education, relapse prevention interventions and support, as well as access to psychological help
7. To offer support and advice around work and education and to refer to specialist services, The role of the CPN is to carry a caseload of clients, to actively work to engage with and support them and to offer an early intervention in psychosis care package in accordance with NICE guidelines. This will include conducting delegated responsibilities on behalf of the London Borough of Hackney, such as Care Act assessments and responsibilities associated with safeguarding adults and children. Clear and timely recording on the clinical record, plus the preparation of statutory and other reports, are essential. CPNs will also be responsible for the administration of depot medication for the relatively small number of patients on depots and other clinically appropriate interventions. They contribute to the team duty rota, which supports both EQUIP and Heads Up. Participation in quality initiatives is encouraged and CPD and reflective practice are embedded in the team.

+ Patients, their networks and carers
+ Manager, supervisor & other colleagues within EQUIP
+ Colleagues within the Early Intervention inpatient ward
+ Colleagues within the Neighbourhood Teams, Assessment and Neighbourhood Recovery Services, and other relevant community teams
+ Partner agencies, including primary care services, local authority services and voluntary agencies
+ Colleagues within other services, such as the Crisis Pathway and AMHP Services
+ Staff in other NHS Trusts

It also works closely with the Heads Up Team, the early detection service for people who are at risk of developing psychosis. The post holders will manage a caseload of patients recovering from a first episode psychosis as well as rotate onto a duty system. The team work to NICE guidelines and within relevant legislation and policy. The post holders will work with other MDT members and be expected to develop their social, biological and psychological understandings of psychosis by attending in-house CPD/group supervisions and co-working with families., Our mission is to make a positive difference to people's lives by improving the quality of life for all we serve.
