Full Job Description

The CSH Diabetes Specialist Nursing team provides specialist clinical advice and care, supporting the delivery of all commissioned activities to adult patients within the North West Surrey area and delivering an integrated model of diabetes services.

The role of the Diabetes Specialist Nurse (DSN) service is to promote the health and wellbeing of people with diabetes enabling them to achieve optimum glycaemic control through informed self-management.

The post holder will provide a link between GP and consultant led specialist teams and will provide education and support both to people with diabetes and those who care for them.

The post holder will be responsible for facilitating improved diabetes care through the provision of advisory services, including telephone, video and face to face support within practices, clinics in an acute setting, nursing homes and patients own homes.

The post holder will be responsible for contributing to the ongoing development of clinical practice, research and standards of care within the service.
