Full Job Description

The aim of the discharge facilitator role is to deliver high standards of patient care and pathway management in order to reduce delayed transfers of care across the trust, using the intermediate care model. The individual must believe and champion that where possible the best place for a patient to be discharged to is their own home

Must have the ability to communicate effectively with the multidisciplinary team whilst having the skill of providing reassurance for both patients and their families during what can be an anxious time when discharge planning

To deliver high standards of patient care and pathway management in order to reduce delated transfers of care across the trust ensuring a safe patient centred discharge

The post holder is required to work on the weekend and bank holiday roster as part of their contracted hours, Ability to work as part of a team liaising with the MDT regarding discharge planning for patients and the timely early identification of potential discharge delays

To assist in communicating the discharge process to the patient but not considered responsible for making the recommendation request

Ability to work independently in order to facilitate discharges in a timely fashion and to highlight any concerns to the discharge hub

Ability to liaise sensitively with patients family and or carers concerning issues related to discharge planning that are appropriate to the grade and assist in identifying and supporting self funded issues restarting packages of care and supporting care homes to be proactive in timely discharges

More complex discharge planning will be supported by seniors

Liaise with other agencies patients family and or carers in order to provide effective communications to assist with timely discharge of patients

Work with the discharge hub lead to provide accurate anticipated discharge lists

Work supportively with the discharge hub colleagues to promote effective discharge communications with the pateints and family as well as ward staff

Have full understanding of new working practices when updates released in conjunction with current infection control guidance

To have an awareness of patient flow throughout the hospital and impact of discharge within this

To have an awareness of criteria to reside policy and the impact of this with discharge planning and patient flow
