Health Care Support Worker @Home

An exciting opportunity has arisen for a Health Care Support Worker to join the @Home Service in a permanent position. The post holder will work as a member of the @Home Service under the direction of the Team Leader. The post holder will contribute to the implementation of nursing care as assessed and prescribed by the Registered Nurse (RN), undertake a range of delegated duties to meet the needs of specified adults in a variety of settings including patients' homes, clinic settings, residential and nursing homes. This includes working in a multi-disciplinary and multi-agency arena and in partnership with the primary health care team. The range of responsibilities will be in support of and supervised by a Registered Nurse or Therapist, and will involve rotation as appropriate to the needs of the service. The post holder will acknowledge their limitations and actively seek advice as necessary., An exciting opportunity has arisen for a Health Care Support Worker to join the @Home Service in a permanent position. The post holder will work within the @Home Service, which provides a comprehensive multi-disciplinary assessment service for adults presenting with complex needs in the community who are likely to be admitted to hospital in the absence of a timely health intervention. The post holder will work closely with multidisciplinary team colleagues, contributing to the implementation of nursing care as assessed and prescribed by the Registered Nurse, undertake a range of delegated duties to meet the needs of specified adults in a variety of settings including patients' homes, clinic settings, residential and nursing homes. We are looking for a skilled, caring professional with a commitment to excellence in nursing practice who will support the clinical leads with the monitoring of the @Home service performance and practice developments. The @Home team will provide a friendly and welcoming environment within a supportive management structure that promotes ongoing educational development.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
