Full Job Description

The post holder will support the Southwest London ICS Learning from Lives and Deaths - People with a learning disability and autistic people Programme ( LeDeR ) with the completion of LeDeR reviews.,

  • Complete LeDeR reviews according to the standardised review process to the highest standards focusing on service improvement and adhering the to eight key principles of the LeDeR programme.
  • Liaise with bereaved families/carers following the death of a person with a learning disability to ensure the family can contribute effectively to the review.
  • Identify, collect and collate health and social care records from social care, hospitals, GPs and other sources.
  • Provide oversight and quality control for reviews completed by members of the reviewing team.
  • Liaise with the LAC for the ICSs covered by the team to allocate reviews appropriately.
  • Support the Band 6 in undertaking initial and focus reviews to ensure completion in a timely manner and agreement with ICB LACs.

    It is commissioned by NHS England and is part of the NHS Long Term Plan. The LeDeR programme has been running since September 2017, co-ordinated and delivered by SW London ICB with the support of a wide range of partner organisations., NHS South West London Integrated Care Board decides how the South West London NHS budget is spent and develops plans to improve people's health, deliver higher quality care, and better value for money.
