Full Job Description

Assess andprovide high standards of initial assessment input to patients in thecommunity. Liaise with MDTmembers in treatment and management care plans and working closely Support OT / PTwith the recommendations for patient centred approach to achieve their rehabgoals Create, evaluate allcare plans implemented and should have a sound knowledge in discharge planningand ability to make further referrals in to the community as required, Assess andprovide high standards of initial assessment input to patients in thecommunity.
