Full Job Description

The role is for the post holder will travel across ABUHB in the community to assess and treat patients in their own homes. The post holder will work alongside a HCSW , the team consist of band 6 triage nurses that triage's the visits and places them on the Civica the computer system used by the overnight team. the team is supported by the team Manager., Assess and deliver care to patients in the community under the direction of the team leader.

Maintain accurate record keeping and standards for documentation including IT systems

Ability to work for a 24 hour community service that would incorporate 12 hour nights

Have competence in a range of clinical skills needed for community nursing.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
