Full Job Description
The post holder will be the point of contact for all administrative issues relating to patients' pathway of care. The post holder will work as part of a multi-disciplinary team and will provide support in the proactive management of the patient pathway from referral to discharge, ensuring the entire pathway is managed smoothly., The successful candidate will work directly alongside the Multi-Disciplinary Team (MDT), to safely and efficiently manage and care for acutely unwell patients remotely. They will act as a point of contact to manage patient interactions and communications, and facilitate assessments, treatments and escalation to the appropriate member of the MDT. You will be responsible for tracking patients across their journey and documenting referrals interactions and communication within the EPR. Will be responsible for coordinating the distribution and delivery of pharmaceuticals, patient information and medical equipment via our logistic services. Demonstrate high levels of compassion and be an ambassador for excellence in patient experience. Undertake clinical audit, whilst managing patient level data through our EPR and database systems. Support the Service Manager in the development and delivery of new pathways and practices., The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
- NVQ Level 3 in Business Administration/Customer Care or acquired equivalent experience
- GSCE (or equivalent) in English Language Desirable criteria
- Customer service qualification or course completion, Experience within the NHS administration environment
- Ability to organise, prioritise workload and delegate
- Use of patient and hospital databases Desirable criteria
- Experience of managing people
- Experience of mentoring
- Experience of audit and data collection, Able to communicate clearly in English
- Demonstrates initiative and enthusiasm in approach to work
