Full Job Description

Phoenix Futures is looking to recruit a passionate and solution focused Planned Care Team Manager at HMP Pentonville.

As the Planned Care Team Manager, you will oversee the effectiveness of care given to service users after 14 days in custody, up to 12 weeks prior to release., within the substance misuse service. This will include the effectiveness of various groups, packages, one to one sessions and multidisciplinary/partnership work.

You will draw on expertise to provide a knowledgeable and skilled service with the ability to draw upon specialist input rapidly when needed. You will support and lead Recovery Workers, ensuring they provide support, continuity and coordination for people who use our service. You will ensure that individuals are accessing the relevant services available in the prison and that there is a plan in place for each prisoner to support their long term engagement with healthcare and the management of any health condition they might have.

As an individual, we are looking for someone who is self-motivated and good problem solver who can work on their own initiative, and as part of a team. You will need excellent communication skills and the ability to navigate challenging situations with a solution focused approach. The ideal candidate will have experience of line management, safeguarding and working with vulnerable adults presenting with substance, alcohol and associated issues.

This role will be rota based working early and late shifts, and Monday-Sundays on a rolling rota basis
