Full Job Description

The role will be Full Time and will involve assessing and providing support to clients whom have moderate mental health needs alongside GPs and practice staff. Your will form productive working relationships with our local GP colleagues and provide clinic time within general practice - whilst being supported by a friendly and knowledgeable mental health team.

The well-established Primary Care Plus (PCP) in Milton Keynes is seeking an enthusiastic, dynamic and motivated Band 6 mental health professional to cover a fixed term period of Secondment., You will need to have excellent assessment skills, be able to work autonomously and have a working knowledge of secondary mental health care and the mental health needs of dynamic and changing population.

Detailed job description and main responsibilities
The post holder will be a member of the Primary Care Plus team. .This post has the responsibility for providing effective day to day mental health care and management to clients with mental health problems presenting in Primary Care. The PCP team provides a single pathway for assessment, referral management and care coordination on behalf of patients with more complex mental health problems presenting in Primary Care.
The post holder will offer short term mental health interventions to support this client group within primary care settings and will work at the primary secondary care interface to facilitate movement of clients between primary and secondary care services. PCP provides a dedicated service package to support those newly discharged from secondary care and their GP practice. A substantial part of their working time will be based within the GP practices of the designated patch.
