Full Job Description

Responsible for the delivery of shared care pathways in line with cancer standards, as well as data reporting for said pathways, in conjunction with the Cancer Services Manager and Cancer Performance Manager, as well as any other requests required to support the analysis of the success of the role.

To work with colleagues across the Cancer Alliance to co-ordinate cancer pathways and to ensure efficient and accurate data collection for shared patients., To co-ordinate the cancer pathways for key tumour sites across the Cancer Alliance. Ensuring that all cancer patients referred via all cancer pathway routes are treated within National Cancer Waiting Times targets to ensure delivery of subsequent standards (if appropriate)

Supporting shared care patients through the cancer pathway as required by the patient when they are referred to the service from across the SSCA region.

Navigation of patients through the cancer pathway, ensuring timely booking and reporting of investigations, management and escalation of delays and issues in line with the optimal timed pathway and agreed KPI's

Accurate and timely recording of all aspects of the patient's pathway from receipt of referral to either discharge or onward referral/treatment

Advocating on behalf of the patient by timely liaison and co-ordination with colleagues in primary and secondary care to ensure timely navigation

Ensuring that all patients identified for shared care are diagnosed within National Cancer Waiting Times standards.

Oversight and tracking of the process of diagnosis whilst working closely with the Validation Cancer Services Team Leader to ensure the collection, verification, and input of complete and accurate data for the purpose of meeting Cancer Outcomes & Services Dataset (COSD)
